These are North Texas’ most unsafe hospitals for spring 2025, says list. See the ratings

A new hospital safety report highlights how good North Texas hospitals are at protecting patients from preventable harm.

The Leapfrog Group, a nonprofit that tracks patient safety nationwide, just released its Spring 2025 Hospital Safety Grades. The results aren’t exactly comforting for the Dallas-Fort Worth area.

Twenty-four hospitals in the North Texas region received a “C,” and five were given a “D” in the list.

The good news? No D-FW hospitals received a failing “F” grade.

Here’s what you should know.

How did experts determine hospital safety?

Leapfrog assigns hospitals a letter grade from A to F twice a year based on their ability to prevent medical mistakes, injuries, infections and other harm. (You can find the D-FW results for fall of 2024 here.)

The grades are based on up to 30 performance measures collected from the Centers for Medicare & Medicaid Services (CMS), the Leapfrog Hospital Survey and other supplemental sources.

Together, those metrics create a single safety score. The goal? To help patients make more informed decisions about where to seek care.

According to Leapfrog, preventable medical errors and complications harm one in four hospital patients and contribute to up to 250,000 deaths annually.

How safe are North Texas hospitals?

In the Dallas-Fort Worth area, 24 hospitals in Leapfrog’s study received a “C” grade, meaning they fall in the middle range of safety performance. Five others received a “D,” putting them closer to the bottom.

Even with these lower grades, some hospitals with “C” rankings, such as Baylor University Medical Center, Medical City Dallas, and Texas Health Southwest Fort Worth, were named by Newsweek as among the best in Texas.
