The latest round of one group’s hospital safety grades are in for North Texas.
The Leapfrog Group, a national nonprofit that tracks hospital safety twice a year, just released its Fall 2025 report, grading hospitals across the country on how well they protect patients from medical errors, infections and accidents.
While some hospitals kept their top scores from the spring, some saw their grades slip.
Here’s what to know.
How are hospital safety grades determined?
Leapfrog assigns each hospital a letter grade from A to F based on more than 20 measures of patient safety.
Those include infection rates, communication with staff, error prevention and outcomes like falls and surgical mistakes.
The grades are updated twice a year, in spring and fall, and are meant to give patients an easy way to compare how safely hospitals operate.
How did North Texas hospitals perform this fall?
This fall’s grades show a wide range of results across Dallas-Fort Worth hospitals.
Several hospitals maintained their A grades from previous cycles, while others slipped to Bs or Cs.
