An Azle man killed in a head-on collision on a two-lane road was identified Thursday.

Donald Hays, 45, died Wednesday afternoon at the scene of a wreck at south 13000 Farm-to-Market Road 730 about a mile south of Jacksboro Highway in Azle, according to the Tarrant County medical examiner’s office.

Azle Police Chief Rick Pippins said Hays was driving a Ford F-150 pickup northbound at about 4:18 p.m. when he swerved into the southbound lane and collided with an oncoming Ford F-250 pickup. The driver of the F-250 suffered “numerous injuries” and was taken to Harris Hospital in Fort Worth.

The accident was still under investigation Thursday, Pippins said.

“There's nothing right now that indicates what happened, no skid marks or anything of that nature,” Pippins said. “It was one of the things where the individual crossed the line. There’s so many different things that could have caused that.”
