Pre-Kindergarten

Vaccine – Required Doses

Diphtheria, Tetanus, Pertussis (DTaP) 4 doses

Polio (IPV) 3 doses

Measles Mumps, Rubella (MMR) 1 dose on or after 12 months of age (1st birthday)

Haemophilus Influenzae type b conjugate vaccine (Hib) 1-4 doses. 1 dose if given on or after 15 months OR 3-4 doses (Completed series with the last dose given on or after 12 months of age)

Varicella 1 dose on or after 12 months of age (1st birthday) if no history of chickenpox

Hepatitis A 2 doses

Hepatitis B 3 doses

Pneumococcal Conjugate Vaccine (PCV7) 1-4 doses. 1 dose if given on or after 2nd birthday OR 2 doses if both given on or after 12 months of age and 8 weeks apart OR 3-4 doses if last dose was given on or after 12 months of age (1st birthday)

Kindergarten through 12

Vaccine – Required Doses

Diphtheria, Tetanus Toxoid, and Pertussis Vaccine (DTaP/DTP/ DT/Tdap/Td) Five doses; however, 4 doses meet the requirement if the 4th dose was given on or after the 4th birthday.

Students 7 years or older

Three doses of any combination Tdap/Td/DTP/DTaP

One dose of Tdap/Td required ten years after last dose of DTP/DTaP/DT.

Polio (IPV) Four doses; however, 3 doses meet the requirement if the 3rd dose was given on or after the 4th birthday.

Measles Mumps, Rubella (MMR) Two doses; both on or after the first birthday.

Hepatitis B Three doses are required.

Varicella One dose on or after the 1st birthday. (Two doses if vaccine given at 13 years of age or older.)

Immunizations are available for $10 (or bring current Medicaid letter) at the following locations:

Six Flags Mall, Arlington; Friday-Wednesday, Aug. 8-13, 10 a.m. to 6 p.m.; and Thursday-Saturday, Aug. 14-16, 10 a.m. to 8 p.m.

La Gran Plaza, Fort Worth; Monday-Saturday, Aug. 18-23, 10 a.m. to 6 p.m.; and Monday-Friday, Aug. 25-29, 10 a.m. to 8 p.m.

Back-To-School Round Up, Will Rogers Complex; Saturday, Aug. 23, 9 a.m. to 4 p.m.

No-cost immunizations will be available to eligible children (uninsured, underinsured, on CHIP or have Medicaid) on a first-come, first-served basis at the following locations:

Fire Station #3, 4700 Ramey; Mondays, Aug. 11, Aug. 18, Sept. 8, Sept. 15, Oct. 20, Nov. 10, Nov. 17, Dec. 8 and Dec. 15, 6 p.m. to 9 p.m.

Fire Station #15, 3100 Azle; Sundays, Aug. 10, Sept. 14, Oct. 12, Nov. 9 and Dec. 14, 1 p.m. to 4 p.m.; and Thursdays, Aug. 28, Sept. 25, Oct. 23, 6 p.m. to 9 p.m.

Fire Station #17, 212 E. Felix; Sundays, Sept. 7, Oct. 5, Nov. 2 and Dec. 7, 1 p.m. to 4 p.m.; and Thursdays, Aug. 21, Sept. 18, Oct. 16, Nov. 20 and Dec. 18, 6 p.m. to 9 p.m.

Medical Care

Please keep all emergency contact numbers and names on your child’s enrollment card current. The school nurse or designated staff will administer all medications/procedures when there is a written order from the authorizing physician and written authorization from the parent/guardian.
