In states that refuse to set up the insurance exchanges under the Affordable Care Act, like Texas has, and the federal government is forced to set up the exchanges, the only option on the government-established exchanges should be Medicare -- no private insurance companies. Having younger, healthier individuals in the Medicare system would help to stabilize Medicare and extend it further.

Besides, it would point out to both voters and private insurance companies that those "red state" governments are not working.

This isn't about politics -- that's what the "refuser states" are doing. To the contrary, it's a workable solution to a problem being created by those who are playing politics with our access to healthcare. Almost one of every four Texans does not have health insurance and therefore must use the emergency rooms, driving up the cost of healthcare for all.
